Middle Georgia State University’s top priority is the health, safety and well-being of our university community. All students, faculty, and staff are asked to register their travel plans via this form. This information may be used to inform campus decisions regarding health and safety, or to contact you if necessary.
Please check the travel information on the CDC and State Department websites and talk with your family/travel companions to make an informed decision about travel plans.
The CDC strongly advises that you should not travel to a location with a Level 3 or Level 4 Travel Health Notice. All travelers originating from, returning from, or connecting through a CDC Warning Level 3 or higher travel notice area MUST NOT return to a Middle Georgia State University campus or facility until they have been asymptomatic for a period of 14 days from the date of entering/reentering the United States.
