Student Tax Notification
**** 1098-T tax notification will be available by January 31st ****
An electronic 1098-T Tuition Statement form will be on SWORDS. Once the information is available, the student can access it on SWORDS by following these steps:
- SWORDS Secure Login.
- Under 'Student Accounts'
- Click the Tax Notification (1098-T) link, and select the Tax Year.
Please note: Students whose amounts are zero for the 1098-T boxes 1, 4, 5, and 6 are excluded from the data provided to TSC and to the IRS. Examples of students who might be excluded are senior citizens and USG employees using tuition assistance.
If you believe the information reported is not accurate please follow the instructions below to request a review (please keep in mind, Box 1 of the 1098-T form only includes payments received for what the IRS determines to be Qualified Tuition & Related Expenses (QTRE). Not all of your charges qualify, for example, housing, meals, books are not considered qualified tuition and related expenses, therefore, it will not be reflected in Box 1):
- Review the definitions of the 1098-T form and reporting rules established by the IRS.
- Prepare the information you believe should be reported and provide the documentations for your reasoning.

IRS Compliance Requirement
In order to comply with IRS requirements to report tuition payments and other information on Form 1098-T, Middle Georgia State University (MGA) needs to collect students' taxpayer identification numbers (TINs). For most students, their TIN is their Social Security number (SSN). Students who are not eligible for an SSN may have an individual taxpayer identification number, known as an ITIN. Students are required by law to provide the TIN so that it may be included on an information return.
At least once a year, MGA will solicit any missing TINs from students. MGA has created a substitute Form W-9S for the purpose of obtaining this information. Students receiving notification that the TIN is missing should complete the Request for Student's Taxpayer Identification Number (Substitute Form W-9S) and mail or deliver the completed form to the Registrar's Office. Do not email the form since email is not a secure way to transmit personal information.
